AUO Corporation (hereinafter referred to as “AUO”), in accordance with the implementation of the Personal Data Protection Act of Taiwan hereby requests you to fill out this Consent Form to give your consent to AUO to use your personal information in conformity with the Personal Data Protection Act. The specific contents of this Consent Form are as follows: I hereby agree that AUO can collect, handle or use my personal information (including but not limited to name, cellphone number, company, job title, e-mail) within the scope of handling the FindARTs related consulting services and other related tasks (“Purpose”).  AUO is hereby allowed to legally use my personal information pursuant to the Purpose and in compliance with Personal Information Protection Act and its relevant stipulations, until the elimination of all the Purposes set forth herein. 

I understand that, as provided under the Personal Information Protection Act, I have the right to inquire about, review, obtain a copy of, request to stop further collection, processing, or use of, delete, supplement or correct, my personal information held by AUO by submitting a written request to AUO.  If AUO is unable to do the aforesaid collection, processing or use, AUO may not be able to proceed with necessary handling the FindARTs related consulting services and other related tasks.
